Back in the day, when I used to GM Call of Cthulhu and AD&D, I would dream up richly layered and detailed plots and characters, that were then ignored or completely misread by the players in 90% of cases. Show some people a plot device and they will either want to shoot it, burn it or ignore it.

If only we had had dynamite as 1st level characters encountering stirges in a basement in the City State of The Invincible Emperor...

I had- and still have- lots of city state stuff. I prefer invincible Overlord to world emperor though, it feels more ramshackle and flavoursome
