Are they fanatics because they have tiny ones? I assume having a tadger that size would make you a bit warped.

I'm doing a full group of 10 Gesatae (sp). Maybe 2. If I've read the rules correctly, the fanatics ambush from within another group, so 6 more nude dudes would pop out unexpectedly from the mob already on the table.

yep, that's one of the ways naked fanatics can appear. And if you time it right the one-shot weapon can do some damage.
